Step 2: Cut the Pages

Cut the page to form individual pages of a Passport.

Step 3: Attach the Pages

Attach the pages together to form a Passport booklet.

Step 4: Assemble the Booklet

Staple pages together, glue the page edges to form booklet, or use plastic binder material.

Step 5: Fill in Answers

Using a pen or pencil – fill the answers to all the questions.

Step 6: Write Your Name

Write your name on the first inner page.
